Requests for documentation and authorisation
To request a copy of a medical record, reports, or complementary tests, you need to:
Send the application form, which you can find here as a PDF, to the Documentation Department at Clínica Girona.arxiu@clinicagirona.cat) duly completed and signed. The application indicates the documentation that must be attached.
Once we receive the application form, we will reply to the same email with the encrypted documentation and, in a second email, you will receive the password that will allow you to access it.
