Customer Service is located at the fourth floor from the Clinic, at 206 Barcelona Street. Their public opening hours are Monday to Thursday from 9:00 AM to 5:00 PM and Fridays from 8:00 AM to 3:00 PM.

Requests for documentation and authorisation.
To request a copy of your medical history, reports, or additional tests, you need to:
Send the application form, which you can find here as a PDF, to the Documentation Department at Clínica Girona.arxiu@clinicagirona.cat, duly completed and signed. The application indicates the documentation to be attached.
Once we receive the application form, we will reply to the same email with the encrypted documentation, and in a second email you will find the password which will allow you to access it.
