Clínica Girona’s Controller, in compliance with Regulation (EU) 2016/679 of the European Parliament and of the Council, of April 27, 2016, regarding the protection of natural persons with regard to the processing of personal data and the free circulation of these, ahead RGPD, informs you that it is adhered to the Standard Code of the Catalan Hospital Union (UCH), with the CT151 registry, in compliance with Organic Law 15/1999, on the Protection of Personal Data (LOPD ), and that it has a Data Protection Delegate who ensures the correct processing of your data, which will only be used to fulfill the purpose and objectives for which it has been provided. Girona Clinic will treat these in accordance with legal requirements, taking the corresponding security measures to guarantee the confidentiality and integrity of the information in accordance with the provisions of the new RGPD and other applicable legislation.

Clínica Girona informs you that you have the power to exercise the rights of access, rectification, deletion, limitation of treatment, portability and opposition of your data in accordance with the provisions of the regulations on data protection. To exercise them, you must write to the Data Protection Officer at C/Barcelona 204-206 (17005-Girona) or by email to protecciodades@clinicagirona.cat; In any case, a photocopy of your national identity document or equivalent must be attached. Likewise, you are informed of your right to file a claim with the competent control authority, in accordance with the corresponding procedure according to the specific case.
